Medical Alert Cards

Our medical alert cards are a free service for members of CAUK, over the age of 18.

Each card is personalised with a colour photo, full name and emergency contact details.

About our Medical Alert Card Scheme

Medical Alert Cards are free to our members and upon request will be printed and posted to you within 1 to 2 months by the volunteer who kindly manages this service for us.

Each card will be personalised with a colour photo and full name. There will be facility to choose from a number of statements that best relate to the card holders’ symptoms. There will also be a free field where the holder can add a personal statement, unique to themselves. An emergency number may also be added.

In the top right-hand corner there is a QR code. When scanned this links to the Fast Facts section on the CAUK website. This is a very versatile function and will be developed further in the future.

The rear of the card has details relating to CAUK and a return address should the card be lost.

Our medical alert cards are currently available to all members in the UK over the age of 11 who have cavernoma*.  Cards will be provided free of charge.  Please note, due to high demand the processing time for new medical alert cards is 1 to 2 months. 

* If you would like to request a card for your child under the age of 11 please contact us.

This is not an ID card. It is designed to give the holder confidence that they would be able to receive medical assistance in an emergency.

On the front of the card, 

  • Your full name
  • A picture of you
  • A QR code link to an information page
  • Four customisable statements about your symptoms and how they impact you
  • An emergency contact number

On the back of the card, 

  • Our helpline number
  • Our email and website address
  • Postal address for returning the card, if lost

Supporting CAUK

While our Medical Alert Cards are free, we do ask if people could donate if able. This helps offset the cost of the project, and helps add to the scheme in the longer term.
